Connect4Health began in 2014 as a collaboration in Melbourne’s Inner East region, to generate community impact by sharing a vision for improved health and well-being. Carrington Health and healthAbility partnered with Link Health, Inspiro and Access Health and Community, to work together on primary healthcare service delivery and planning. Connect4Health respond to challenges in our community with a focus on three main objectives; 1. Improve equity and access to services 2. Support community partnerships and 3. Share resources and expertise.
The First Stop: Drug and Alcohol Support For Families and Medication Support & Recovery Service are one of many projects delivered by the Connect4Health collective.
Eastern Health provides a comprehensive range of high quality acute, sub-acute, palliative care, mental health, drug and alcohol, residential care and community health services to people and communities that are diverse in culture, age, socio-economic status, population and health care needs.
The Eastern Metropolitan Region (EMR) is one of eight Department of Health regions and covers Manningham, Booroondara, Yarra Ranges, Knox, Monash, Whitehorse and Maroondah communities. The EMR Department team work closely with service partners to provide high quality health and aged care services. Their ambition is to enhance and protect the health and well-being of all Victorians.
Primary Health Networks (PHNs) have been established with the key objective of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.
The Inner East Primary Care Partnership (IEPCP) is a voluntary alliance of health and human services organisations. The partnership aims to work together towards better planning, coordination and service delivery to improve the health and well-being of people living in Boroondara, Manningham, Monash and Whitehorse communities.
We offer a wide range of services and aim to support the health and wellbeing of all people in our community. This means we provide a combination of both private and publicly funded health care options. We deliver under the following government initiatives- Community Health Program, Early Intervention in Chronic Disease, Home and Community Care, Community Health Support Programme and Vichealth.
