Summary of Our Services

CH provides services and programs funded via a variety of funding sources. These include:

Community Health Program (CHP) – which incorporates Early Intervention in Chronic Disease (EICD) – including a range of allied health services and programs; health promotion activities; and our chronic disease management program

Home and Community Care (HACC) – providing a range of targeted allied health services, delivered in centre and on a domiciliary basis.

Vic Health – A world-first health promotion foundation, VicHealth focuses on promoting good health and preventing chronic disease.

What is the eligibility criteria?

Services are available to people who work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care cards or Pension cards. Some of our services are provided for particular age groups, health conditions or disabilities, such as services to help frail older people and their carers continue managing independently in their homes; or services for young children (0-6 years).
