Community Home Support Program
The Access & Support Program provides personalised support for people who wish to arrange Community Home Support Program (CHSP) and other services so they can stay at home. CHSP provides basic support to people to help them to:
- Stay living at home
- Be active
- Be connected to their community
- Support carers and families
The Access and Support facilitator will talk with you about:
- What is important to you (your needs and goals)
- What you need help with to stay safe and well at home
- What you want to be able to do or keep doing
- The range of support services that are available in your area
The Access and Support facilitator will work together with other people who support you such as family members, friends, your community, your doctor and other health professionals.
Who can use this service?
Access & Support program can be utilised by people who need help to find services that meet their needs and are frail, older or younger with a disability. Needs are related to dementia, language, culture, religious background, financial disadvantage, lifestyle and or risk of homelessness. They also need to reside in the cites of Boroondara, Manningham, Monash or Whitehorse.
What does it cost?
There is No Charge for the Access and Support program.
